Testosterone for Women in Menopause: What the Evidence Actually Shows

Testosterone is prescribed for menopause libido, energy and brain fog — but only one of those has good evidence. Here's what the research actually supports.

What is testosterone actually approved for in women?

Nothing, in the United States — and that is the first thing worth understanding, because it shapes everything that follows.

There is currently no testosterone product approved by the FDA for use in women at female physiological doses. This is not because testosterone does not work in women; it is a regulatory and commercial history. A female testosterone patch called Intrinsa was approved in Europe in 2006 and later withdrawn from the market for commercial reasons. In the US, an FDA advisory committee declined to approve it over long-term safety data concerns. Since then, no manufacturer has brought a female-dose product through approval.

What exists instead is off-label prescribing. Clinicians use male-approved testosterone gels and creams, dispensed at approximately one-tenth of a typical male dose, with regular blood monitoring to keep levels in the premenopausal physiological range rather than allowing them to drift into male range.

The 2019 Global Consensus Position Statement on the Use of Testosterone Therapy for Women exists precisely because of this gap. Published simultaneously in four journals and endorsed by eleven international societies — including the International Menopause Society, the Endocrine Society, and the International Society for the Study of Women's Sexual Health — it was written to give clinicians clear guidance in the absence of regulatory approval and appropriate dosage forms.

Its central conclusion is narrow and specific: the only evidence-based indication for testosterone therapy in women is hypoactive sexual desire disorder (HSDD) in postmenopausal women. Everything else — energy, mood, cognition, muscle mass, bone density, general wellbeing — was judged to have insufficient evidence to support a recommendation.

That is a much narrower position than testosterone's popular reputation in menopause circles suggests, and it is worth holding onto as you evaluate what you read and hear.

Does testosterone actually improve libido in menopause?

For postmenopausal women with HSDD, yes — this is the one area where the evidence is genuinely solid.

Randomized trials and systematic reviews of transdermal testosterone in naturally and surgically menopausal women with HSDD found the testosterone group had significantly more satisfying sexual episodes, more frequent sexual activity, more orgasms, greater desire, and significantly improved scores on the Personal Distress Scale compared with placebo. That last measure matters: HSDD is defined not just by low desire but by low desire that causes personal distress, so improvement in distress is the outcome that reflects whether a woman's actual problem got better.

The effect size deserves honesty, though. The average improvement is real but moderate — typically on the order of one additional satisfying sexual event per month over placebo in pooled analyses. Placebo response in sexual function trials is notably large, which is part of why the drug-placebo difference looks smaller than individual women's experiences might suggest. Some women report a substantial change; others notice little.

A few important boundaries. The evidence is for postmenopausal women. Data in premenopausal and perimenopausal women is much thinner, and the consensus statement did not extend its recommendation to them. The evidence is also for transdermal delivery — patches, gels, creams. Oral testosterone is not recommended in women because of adverse effects on lipid profiles, particularly HDL cholesterol.

And context matters enormously. Low desire in midlife is frequently multifactorial: vaginal dryness and painful sex, relationship strain, exhaustion, depression, and the side effects of SSRIs all suppress desire independently. Genitourinary syndrome of menopause is a particularly common and highly treatable cause — local vaginal estrogen resolves it for most women, and it is not the same conversation as systemic testosterone. Treating pain before treating desire is usually the right order.

Its conclusion across all of these was that the evidence is insufficient to recommend testosterone therapy. Not that testosterone definitively does not help — that the studies needed to know either way have not been done at adequate size, duration, or quality.

That distinction is worth sitting with, because it cuts both ways. It does not mean women reporting improved energy on testosterone are imagining it. Individual responses vary, and the absence of trial evidence is not evidence of absence. But it does mean that a clinic promising testosterone will fix your brain fog, restore your energy, and rebuild your muscle is making claims that go well beyond what the evidence base supports — and they may be charging you a great deal for them.

It is also worth considering the alternatives that *do* have evidence for those specific complaints. Brain fog in menopause often improves with treatment of the sleep disruption and vasomotor symptoms driving it. Fatigue may reflect low ferritin, thyroid dysfunction, sleep apnea, or depression — all testable and treatable. Muscle loss responds robustly to progressive resistance training and adequate protein, which have far stronger evidence in postmenopausal women than testosterone does. None of this means testosterone is never worth trying for these symptoms. It means going in with accurate expectations, after the higher-evidence options have been addressed, and with a clear plan to stop if it does not help.

Why shouldn't a blood test decide whether you need it?

Because it does not work, and the consensus statement is explicit on this point: a blood total testosterone level should not be used to diagnose HSDD.

There are several reasons. Testosterone in women circulates at concentrations near or below the reliable detection limit of many standard immunoassays, so the number a routine lab returns may not be accurate at female physiological levels. Levels also fluctuate through the day and, in women still cycling, across the menstrual cycle. And most fundamentally, blood testosterone concentration does not correlate well with sexual desire in women — plenty of women with low-normal levels have entirely satisfying sexual function, and plenty with mid-range levels have distressing HSDD.

This is different from how testosterone works in men, where a low level is the diagnosis. In women, HSDD is a clinical diagnosis based on symptoms, duration, and — critically — the presence of personal distress about it, after other contributing causes have been considered.

So what is blood testing actually for? Monitoring, not diagnosis. Once you start therapy, the consensus recommends measuring testosterone regularly to confirm you are staying within the premenopausal physiological range and not drifting into male-range concentrations, which is where the risk of unwanted effects rises.

This matters practically because a common clinic pattern is to run a testosterone panel, tell a woman her level is "low," and recommend therapy on that basis. That reverses the logic. If you are told your testosterone is low and that is the reason you should start treatment, it is fair to ask what symptoms are being treated and whether distressing low desire is among them.

The flip side is equally important: a normal-looking level is not a reason to deny treatment to a woman with genuine, distressing HSDD. The number is not the gatekeeper in either direction.

What are the risks and side effects?

At correct female physiological doses, testosterone is generally well tolerated — and most reported problems come from doses that were too high, not from testosterone itself.

Common, dose-related, usually reversible: acne or oily skin, increased facial or body hair growth, and mild scalp hair thinning. These typically appear when levels drift above premenopausal range and often improve on dose reduction. Localized hair growth at the application site is common and can be avoided by rotating sites.

Less common but not fully reversible: voice deepening and clitoral enlargement. These occur at supraphysiological levels and are the main reason regular blood monitoring is not optional. Voice changes in particular may not fully reverse after stopping, which is why the consensus panel emphasizes staying within premenopausal concentrations.

Uncertain long-term: cardiovascular and breast safety data beyond about two years is limited, and the consensus statement notes this explicitly. Transdermal delivery at physiological doses does not appear to adversely affect lipids in the way oral testosterone does, but long-duration outcome trials in women do not exist.

The compounding problem. The consensus panel recommended against compounded testosterone, and this is one of its firmest positions. Compounded creams and — especially — subcutaneous pellets deliver unpredictable and often supraphysiological doses. Pellets cannot be removed or dose-adjusted once implanted, so if levels run too high, you wait months. Most of the serious virilization cases reported in the literature involve pellets or compounded preparations. The Menopause Society has issued similar warnings.

Practical monitoring. Expect a baseline level, a recheck roughly 4-6 weeks after starting or changing dose, then periodically — often every 6 months. Give it at least three months before judging whether it is working, and agree with your clinician in advance on what "working" looks like and when you would stop. How do you have this conversation with your doctor?

Prepared, and with realistic expectations — because many primary care clinicians have not been trained in this and may not know the evidence base well.

Lead with symptoms and distress, not with the hormone. "My desire for sex has dropped substantially over the past two years and it genuinely bothers me and affects my relationship" is a much more productive opening than "I'd like my testosterone checked." The first describes the clinical picture HSDD is diagnosed from. The second invites a test that the guidelines say should not drive the decision.

Name what you have already ruled out. If sex is painful, say so — that is genitourinary syndrome of menopause and local vaginal estrogen is the answer, not testosterone. If you are exhausted and sleeping badly, that belongs on the table too. If you take an SSRI, mention it, because sexual side effects are common and dose adjustment or a switch may be the simpler fix.

Ask specific questions. What formulation would you use, and at what female dose? How will you monitor my levels? How long before we know if it is working? What would make us stop? Do you use compounded products or pellets — and if so, why, given the consensus recommendation against them?

Expect that you may need a specialist. Menopause-certified clinicians (the Menopause Society maintains a directory) and clinicians who treat female sexual dysfunction are far more likely to be familiar with off-label female dosing and monitoring. It is entirely reasonable to seek one out rather than push a reluctant generalist.

Be alert to the opposite failure mode. Clinics that prescribe testosterone reflexively to every midlife woman, use pellets, skip monitoring, and promise fixes for energy and brain fog are overselling. Both under-treatment and over-treatment are real problems here.
