What does 'bioidentical' actually mean?
Bioidentical describes a hormone whose molecular structure is identical to the one your ovaries produce. That's it. It's a statement about chemistry, not about origin, safety, or quality.
The two hormones this applies to in menopause care are:
- •17β-estradiol — the main estrogen your ovaries made before menopause.
- •Micronized progesterone — structurally identical to the progesterone your body produced after ovulation. "Micronized" just means the particles were made small enough to absorb properly when swallowed.
Here's what surprises most women: both are available as FDA-approved, mass-manufactured, insurance-covered products. Estradiol patches, gels, sprays, rings and tablets are bioidentical. Prometrium and its generics are bioidentical micronized progesterone. You do not need a compounding pharmacy to get bioidentical hormones.
The word "synthetic" is where things get muddled. In common usage it's meant to signal "artificial and therefore worse," but chemically it just means made in a lab — and virtually all hormone therapy, including bioidentical estradiol, is synthesized in a lab, typically from plant sterols found in soy or yams. The plant origin story you see in marketing is true of the FDA-approved products too.
What people usually *mean* by synthetic hormones are two specific things: conjugated equine estrogens (Premarin, derived from pregnant mare urine, containing estrogens humans don't naturally produce) and synthetic progestins like medroxyprogesterone acetate (MPA), which are structurally different from progesterone. Those are the compounds used in the original Women's Health Initiative trial — and that matters more than the bioidentical label does.
Is bioidentical HRT safer than synthetic HRT?
For progesterone, the evidence suggests a meaningful difference. For estrogen, the delivery route matters more than the molecule.
On the progesterone side, the comparison that matters is micronized progesterone versus synthetic progestins. The E3N cohort — a large French prospective study following over 80,000 women — found that estrogen combined with micronized progesterone was associated with no significant increase in breast cancer risk over the first five years of use, while estrogen combined with synthetic progestins was associated with a clear increase (Fournier et al., 2008). This is observational data, not a randomized trial, so it can't prove causation. But it's consistent, biologically plausible, and has shifted prescribing practice across Europe and increasingly in the US.
Micronized progesterone also has a practical advantage: taken at bedtime, its metabolite allopregnanolone acts on GABA receptors and produces mild sedation, which many women find helps sleep. On the estrogen side, the distinction that actually drives risk isn't bioidentical versus conjugated — it's transdermal versus oral. Oral estrogen passes through the liver first and increases production of clotting factors, raising venous thromboembolism risk. Transdermal estradiol bypasses that first pass and, in observational studies, does not appear to carry the same clot risk. What's the problem with compounded bioidentical hormones?
The issue isn't the hormones. It's that nobody has verified what's in the product or that the dose is consistent.
Compounded bioidentical hormone therapy (cBHT) is custom-mixed at a compounding pharmacy, usually as a cream, troche, or subcutaneous pellet, often based on saliva hormone testing. These preparations are not reviewed by the FDA for safety, efficacy, or manufacturing consistency.
In 2020, the National Academies of Sciences, Engineering, and Medicine (NASEM) published a comprehensive review at the FDA's request and concluded there is insufficient evidence to support the routine use of compounded bioidentical hormone therapy. The report specifically flagged:
- •Dose inconsistency. Testing has repeatedly found compounded products delivering substantially more or less hormone than labeled.
- •Absent safety labeling. FDA-approved products carry required warnings and patient information. Compounded products don't have to.
- •Unproven claims. Marketing of cBHT frequently implies superior safety, which no trial has demonstrated.
Saliva hormone testing, which often drives compounded prescribing, is not supported as a basis for dosing. Salivary hormone levels fluctuate hour to hour and correlate poorly with tissue levels. The Endocrine Society and the American College of Obstetricians and Gynecologists both advise against using it to guide therapy. Pellets deserve specific caution. Implanted every three to six months, they cannot be removed if you have a reaction or the dose is too high, and they frequently produce supraphysiologic estradiol levels — well above what a premenopausal ovary would produce.
There is one legitimate role for compounding: genuine allergy to an ingredient in a commercial product, or a need for a dose or form that isn't commercially available. That's a narrow indication, not a general approach.
Which HRT should you actually ask for?
For most women, the evidence-aligned starting point is transdermal estradiol plus oral micronized progesterone — both bioidentical, both FDA-approved, both usually covered by insurance.
That combination gives you:
- •Bioidentical molecules, if that matters to you
- •Regulated manufacturing and verified dosing
- •Transdermal delivery, avoiding the liver first-pass effect and its clot risk
- •Micronized progesterone, with the more favorable breast signal from E3N and the sleep benefit
If you still have a uterus, you need progesterone. This isn't optional. Estrogen alone stimulates the endometrial lining and raises endometrial cancer risk; progesterone opposes that. Compounded creams marketed as "progesterone" often deliver too little to provide endometrial protection, which is one of the more serious risks of going the compounded route.
Other FDA-approved options worth knowing:

Why is compounded hormone therapy marketed so heavily?
Because the market conditions favor it, and because the language taps into a legitimate frustration.
After the 2002 WHI publication, hormone therapy prescriptions fell sharply and a generation of clinicians stopped being trained in menopause care. Women arrived with real symptoms and were told to wait it out. Into that vacuum stepped clinics offering personalized "natural" hormones, often on a cash-pay basis outside insurance — which also means outside the constraints that come with it. Compounded products, being unapproved, aren't bound by the marketing restrictions that govern FDA-approved drugs.
The frustration those clinics tapped into is real. Many women genuinely were dismissed. But the answer to being dismissed by a system isn't a less-regulated version of the same treatment at four times the price. It's finding a clinician who takes menopause seriously — and there are more of them now than there were five years ago, including telehealth practices focused specifically on midlife care.
Questions worth asking any provider:
1. Is what you're prescribing FDA-approved? If not, why is a compounded version necessary for me specifically? 2. Are you using saliva testing to determine my dose? (If yes, ask why, given that professional societies advise against it.) 3. If I have a uterus, what am I getting for endometrial protection, and is it a dose proven to provide it? 4.
