The risks are real, specific, and worth understanding numerically rather than as a vague warning.

The clearest signal is stroke. Oral estrogen is associated with an increased risk of ischemic stroke, and because baseline stroke risk rises steeply with age, the same relative increase translates into a larger absolute number of events in a 65-year-old than in a 52-year-old. This is the core reason guidelines draw a line at 60 — not because something changes about the drug, but because the background risk it multiplies has grown.

The second is venous thromboembolism (VTE) — blood clots in the legs or lungs. Why would someone start hormone therapy after 60?
