Does a GLP-1 slow down how your body absorbs pills?

Partly - but probably not in the way you're worried about.

GLP-1 receptor agonists (semaglutide, tirzepatide, liraglutide) slow gastric emptying, which is the rate your stomach releases its contents into the small intestine. That slowdown is not a side effect. It's a core part of how the medication works: food stays put longer, you feel full sooner, and blood sugar rises more gently after eating.

But anything you swallow rides along with that food. A pill that used to reach your small intestine - where most absorption happens - in 30 minutes might now take 90.

Cmax is the highest concentration your blood reaches. AUC (area under the curve) is the total drug exposure over time. GLP-1s tend to flatten and delay Cmax while leaving AUC roughly intact. The drug still gets in - it just arrives more slowly.

Two more things worth knowing. First, the slowdown is strongest right after your first dose and after each dose increase, then fades substantially with continued use - a phenomenon called tachyphylaxis. By around 12 weeks on a stable dose, gastric emptying has partially normalized. Second, a 2025 physiologically-based pharmacokinetic modelling analysis (Hooper et al., *Pharmacotherapy*) predicted that for most co-administered oral drugs, the effect stays below the threshold that would change clinical outcomes.

The drugs that deserve real caution are ones with a narrow therapeutic index - where small changes in blood level matter a lot. Levothyroxine, warfarin, some seizure medications and certain antibiotics fall into this group. Menopausal hormone therapy generally does not.

Does semaglutide reduce how much estrogen you absorb?

No - and this has actually been measured directly.

In a dedicated pharmacokinetic trial, healthy women took a combined oral contraceptive containing ethinyl estradiol and levonorgestrel both alone and while at steady state on once-weekly semaglutide 1.0 mg. The finding: semaglutide did not reduce the bioavailability of either hormone. Ethinyl estradiol total exposure was about 11% higher on semaglutide, and levonorgestrel about 20% higher (Kapitza et al., *Journal of Clinical Pharmacology*, 2015). One special case: oral semaglutide (Rybelsus) is its own situation. It relies on an absorption enhancer called SNAC that works in a narrow window in the stomach, which is why the instructions are so strict - take it on an empty stomach with no more than 120 mL of plain water, then wait at least 30 minutes before any other food, drink or medication. Why is tirzepatide different from semaglutide here?

Because tirzepatide actually does reduce oral estrogen levels - and the numbers aren't small.

In the pharmacokinetic study reported in the tirzepatide (Mounjaro and Zepbound) prescribing information, a single 5 mg dose given alongside a combined oral contraceptive reduced ethinyl estradiol peak concentration (Cmax) by about 59% and total exposure (AUC) by roughly 20%. Levonorgestrel showed a similar pattern. Time to peak concentration was delayed by several hours.

Because of that, the manufacturer advises anyone using an oral hormonal contraceptive to either switch to a non-oral method or add a barrier method for 4 weeks after starting tirzepatide and for 4 weeks after each dose escalation.

That warning is about contraception, where a temporary dip in hormone levels can mean an unplanned pregnancy - a high-stakes, all-or-nothing outcome. HRT is a different job. A modest, temporary reduction in absorption doesn't create a hidden danger; it might just mean your hot flashes creep back for a couple of weeks after a dose increase. That's noticeable, and it's fixable.

But here's who should pay close attention: perimenopausal women using a combined oral contraceptive for both cycle control and contraception. That's an extremely common setup in the early forties, and for those women the tirzepatide warning applies in full. Does an estrogen patch or gel avoid the problem entirely?

Yes. Completely.

Transdermal estradiol - patches, gels and sprays - absorbs through the skin straight into your bloodstream. It never enters your stomach, so gastric emptying is irrelevant. Whatever your GLP-1 is doing to your digestion, it cannot touch a patch on your hip.

That's not the only reason clinicians increasingly favour transdermal delivery. Oral estrogen passes through the liver first (first-pass metabolism), which raises production of clotting factors. Vaginal estrogen - cream, tablet or ring - is also unaffected. It acts locally on vaginal and urinary tissue with minimal systemic absorption, and it can be used alongside any of this.

What about progesterone, and does timing matter?

Progesterone is where the real-world issue is most likely to show up - and it's an easy fix.

On a GLP-1, your evening meal may have shrunk to a few bites. If you take your progesterone with almost nothing in your stomach, absorption may be lower and more variable than intended.

What to do: take it with whatever your evening meal is, however small, and try to include a little fat - a spoonful of yogurt, a few nuts, some olive oil on vegetables. Keep the timing consistent night to night. Then watch for the signal that something's off: unexpected breakthrough bleeding, or the return of sleep problems that progesterone had been helping. Either is worth reporting to your prescriber rather than adjusting on your own.

There's also an option that sidesteps absorption entirely: a levonorgestrel IUD provides endometrial protection locally, with no swallowing required. It's licensed for this use in several countries and is a common choice for women who want to simplify.

What should you actually do if you take both?

Here's the practical checklist.
