Do GLP-1s reduce visceral fat specifically?

Does hormone therapy make a GLP-1 work better?
Possibly — the observational signal is interesting, but it has not yet been tested in a randomized trial, and that distinction is important.
A retrospective cohort study examining menopause hormone therapy and tirzepatide-associated weight loss in postmenopausal women found that women on hormone therapy alongside tirzepatide lost meaningfully more weight than those on tirzepatide alone — on the order of 30-35% greater loss in a Mayo Clinic analysis. Women who take hormone therapy differ systematically from women who do not: they tend to have more engaged healthcare, better access, fewer contraindications, and often more health-seeking behavior overall. Any of those could explain part of the difference. This is a signal worth watching, not a settled result.
The practical upshot: if you have bothersome menopause symptoms and no contraindications, hormone therapy is worth discussing on its own merits — and the possible weight-loss synergy is a reasonable point to raise, framed honestly as preliminary. What should you watch for that's different after menopause?
Three things need active management, and none of them are reasons not to take the medication — they are reasons to take it thoughtfully.
Bone density. This is the most important one. Estrogen loss already accelerates bone turnover sharply in the years around menopause, and rapid weight loss independently reduces bone mineral density. Those two effects stack. The mitigations are well established: resistance training and impact loading, adequate protein, and calcium plus vitamin D sufficiency. How should you set expectations for results?

Average results from the pivotal trials: in STEP-1, semaglutide 2.4 mg produced about 14.9% mean body weight loss over 68 weeks. In SURMOUNT-1, tirzepatide produced about 20.9% at the 15 mg dose over 72 weeks. Postmenopausal women in these trials fell within that range. A few things shape where you land. Rate of titration matters — going up slowly usually means better tolerance and better adherence, which beats an aggressive schedule you abandon.
