What did the SURMOUNT reproductive-stage analysis find?
It found that tirzepatide works remarkably consistently across every stage of the menopausal transition — a result that surprised a lot of clinicians who expected postmenopausal women to respond less well.
The analysis, published in *Obesity* in 2025 by Tchang and colleagues, pooled 2,542 women from the SURMOUNT-1, SURMOUNT-3, and SURMOUNT-4 trials and grouped them by reproductive stage. Participants received tirzepatide at 15 mg or their maximum tolerated dose.
The headline numbers:
- •Premenopausal women: 26% average body weight reduction, versus 2% on placebo
- •Perimenopausal women: 23% reduction, versus 3% on placebo
- •Postmenopausal women: 23% reduction, versus 3% on placebo
Waist circumference told the same story: reductions of 22 cm, 20 cm, and 20 cm respectively, against 4 to 5 cm on placebo.
And perhaps the most useful figure for anyone weighing whether to start: between 97% and 98% of participants in every reproductive stage achieved at least 5% weight reduction on tirzepatide, compared with 29% to 33% on placebo. Five percent is the threshold at which clinically meaningful improvements in blood pressure, glucose control, and lipids typically appear.
The three-percentage-point gap between premenopausal and postmenopausal women is real, but it's modest — and it may reflect age and baseline body composition as much as hormonal status, since the groups differed in more than menopause alone. What it clearly does not show is the substantial blunting that many women have been warned about.
The authors' conclusion was direct: tirzepatide provides consistent reduction in body weight and waist circumference across reproductive stages in women with obesity.
Why did anyone expect menopause to blunt the response?
The expectation wasn't unreasonable. Several well-documented metabolic changes at menopause plausibly work against weight loss, and it's worth understanding them even though the trial data came out more favorably than predicted.
Estrogen loss changes fat distribution and metabolism. Estradiol acts on the hypothalamus to influence energy expenditure and on adipose tissue to influence where fat is stored. As it declines, fat redistributes from the hips and thighs toward the abdomen, and visceral adipose tissue — the metabolically active fat around the organs — increases even in women whose total weight is stable.
Lean mass declines. Muscle is the most metabolically expensive tissue you carry. The accelerated loss of it during the menopausal transition lowers resting energy expenditure, meaning fewer calories burned at rest.

Sleep fragments. Vasomotor symptoms and declining progesterone disrupt sleep, and short sleep reliably raises ghrelin, lowers leptin, and increases next-day appetite.
Activity often falls. Joint pain, fatigue, and time pressure in midlife all reduce movement.
Given all that, expecting a blunted drug response was a fair hypothesis. What appears to have happened instead is that GLP-1 receptor agonists act on appetite pathways that are largely independent of the estrogen-driven changes — so they work through a different door. The metabolic headwinds are real, but the medication isn't fighting them directly; it's reducing energy intake through a mechanism menopause doesn't much interfere with.
Does hormone therapy make GLP-1s work better?
The emerging evidence suggests it may — and this is one of the more interesting findings in menopause medicine right now.
A retrospective cohort study published in 2025 examined postmenopausal women with overweight or obesity taking tirzepatide, comparing those who were also on menopausal hormone therapy with those who weren't. Concurrent hormone therapy use was associated with greater weight loss and improved cardiometabolic outcomes.
There are plausible mechanisms. Estradiol influences hypothalamic appetite regulation and energy expenditure, supports lean mass retention, and improves insulin sensitivity. It may also reduce the visceral fat accumulation that menopause drives. In principle, hormone therapy could be restoring some of the metabolic environment that makes weight loss more achievable, while the GLP-1 does the appetite work.
Several caveats matter, though, and they're important:
This was a retrospective cohort, not a randomized trial. Women who take hormone therapy may differ systematically from those who don't — in symptom burden, healthcare engagement, socioeconomic factors, and baseline health. Those differences can produce apparent effects that aren't causal. The finding is genuinely promising, and it is not yet proof.
What it does support is that the two treatments are not in conflict. There's no evidence hormone therapy interferes with GLP-1 effectiveness, and no reason to choose between them if both are clinically appropriate for you. One real interaction worth flagging: GLP-1s slow gastric emptying, which can affect absorption of oral medications, including oral estrogen. Transdermal delivery — patches, gels, sprays — sidesteps that concern entirely, and is often preferred for other reasons anyway. Is the weight you lose after menopause different?
This is the question that actually deserves more attention than the total-pounds question — and where menopause genuinely changes the picture.
All weight loss involves some loss of lean mass alongside fat. Estimates from GLP-1 trials using body composition imaging suggest roughly 25% to 40% of weight lost is lean tissue, broadly comparable to what happens with diet-driven weight loss of similar magnitude.
For a postmenopausal woman, that percentage lands on a smaller starting reserve. Sarcopenia — age-related muscle loss — accelerates after menopause because estrogen supports muscle protein synthesis and satellite cell function. Losing lean mass from an already-declining base is a meaningfully different proposition than losing it at 35.
Bone raises the same concern. Rapid weight loss is associated with reduced bone mineral density, and the first five to seven years after the final menstrual period are already the steepest period of bone loss in a woman's life. None of this argues against treatment. Obesity itself drives cardiovascular disease, osteoarthritis, sleep apnea, and cancer risk, and the SELECT trial (NEJM 2023) found semaglutide reduced major adverse cardiovascular events by 20% in people with established cardiovascular disease and overweight or obesity. The benefits are substantial and well documented.
What it argues for is doing it deliberately: protecting muscle and bone actively rather than hoping they survive the process. That means adequate protein, resistance training, sufficient calcium and vitamin D, and a rate of loss that isn't reckless. What should you do differently if you're postmenopausal?
Five things, and none of them are complicated — they just require deliberate attention rather than assuming the medication handles everything.
Protein first, non-negotiably. Aim for roughly 1.2 to 1.6 grams per kilogram of body weight daily, spread across meals rather than concentrated at dinner. Muscle protein synthesis in older adults responds better to 25 to 30 grams per sitting than to the same total eaten in one go. Resistance training two to three times a week. This is the single highest-leverage habit available. It's the only intervention that reliably signals the body to retain muscle during a caloric deficit, and it loads bone in a way walking doesn't. Start wherever you are — bodyweight, bands, machines. Don't rush the titration. Faster escalation produces more nausea, not faster results, and severe nausea makes hitting protein targets nearly impossible — which is precisely when lean mass losses accelerate. A slower schedule is often the better one here.
Track more than the scale. Waist circumference, strength gains, energy, and sleep quality all capture things the scale doesn't.
