The GLP-1 side. These medications slow gastric emptying — how fast food leaves your stomach. A meal that used to clear in two hours might take four or five. Eat at 8pm, lie down at 10pm, and there is still food in your stomach pressing against a valve that relaxes when you're horizontal. What should you eat in the evening on a GLP-1 in menopause?

Here's why the timing shift works. Appetite suppression on a GLP-1 tends to be strongest in the 24–48 hours after an injection and often strongest later in the day generally. Many women end up eating almost nothing until dinner, then trying to fit their entire day's nutrition into one evening meal — the exact meal that then sits in the stomach at bedtime. The evening framework:
- •Finish eating 3 hours before bed. This is the single highest-yield change. With delayed gastric emptying, the standard 2-hour rule isn't enough.
- •Keep the evening meal moderate in fat. Fat slows emptying further, compounding the medication's effect. Fried and creamy foods are the worst offenders at night.
- •Protein at dinner, not just at lunch. Does treating hot flashes actually improve sleep?

Hormone therapy remains the most effective treatment for vasomotor symptoms, reducing frequency and severity substantially in most women who use it. Sleep improvement typically follows the symptom improvement. What about your dose and injection timing?
Small scheduling changes can meaningfully reduce night-time symptoms without touching your dose.
Move your injection day. If nausea peaks 24–36 hours after your shot, injecting on a Saturday morning means your worst night is Sunday — a work night. Many women shift to a Thursday or Friday injection so the peak lands on a night when a rough sleep costs less. This is entirely within your control and requires no prescription change.
Consider whether you escalated too fast. New or worsening night symptoms almost always show up within two weeks of a dose increase. Rule out the fixable contributors. When should you talk to a clinician about this?
Sleep loss in midlife is common, but "common" is not the same as "nothing to do about it." Raise it with a clinician if any of the following apply.
Snoring, witnessed breathing pauses, or waking unrefreshed despite adequate time in bed. Obstructive sleep apnea is under-diagnosed in women because the presentation often differs from the classic male pattern — women more often report fatigue, insomnia, and mood symptoms rather than loud snoring. Vomiting at night, severe upper abdominal pain, or inability to keep fluids down. These need same-day assessment, not a routine appointment.
Sleep that hasn't improved after four to six weeks of consistent changes. At that point the answer is likely something structural — apnea, a thyroid issue, iron deficiency, or a medication effect — and more discipline won't fix it.
