Why do GLP-1 medications cause muscle cramps?

GLP-1 medications do not cause muscle cramps directly. Cramps are not listed as an adverse event in the major trials of semaglutide or tirzepatide, and there is no known receptor pathway linking these drugs to skeletal muscle contraction. What the medication does do is change your intake so dramatically that the conditions for cramping appear within weeks.

A muscle cramp is an involuntary, sustained contraction of a muscle, most often in the calf, foot or hamstring. The leading explanation is that motor neurons become hyperexcitable when the muscle is fatigued, dehydrated, or working with a shifted electrolyte balance. Every one of those triggers is more likely on a GLP-1.

Think about what changes in the first three months. Appetite drops sharply, so total food volume falls. Less food means less sodium, potassium, magnesium and calcium arriving each day, because in a normal diet those minerals ride along with the meals you are no longer eating. Thirst cues blunt alongside appetite, so fluid intake quietly falls too. If nausea, vomiting or diarrhoea show up during dose escalation, fluid and sodium losses accelerate further.

At the same time, weight is coming off fast, and some of it is muscle. Is it dehydration or an electrolyte problem?

In practice it is usually both, and they are hard to separate because they travel together. The distinction still matters because the fix is slightly different.

Dehydration cramps tend to appear late in the day, after heat or exercise, and come with dark yellow urine, headache, and lightheadedness when you stand. How much water and salt should you actually be getting?

Start with fluid, because it is the fastest lever. A reasonable target for most adults on a GLP-1 is 2.5 to 3 litres of total fluid per day, which includes tea, coffee, soup and the water in food. That is more than many people manage once appetite drops, so it needs to be deliberate rather than intuitive. The practical version: a full glass on waking, one with each medication, one before each meal, and one in the evening. Front-load it, because drinking large volumes with meals worsens the early fullness that GLP-1s already cause.

Urine colour is the simplest feedback loop. Pale straw is the target. Clear and constant means you may be over-drinking and diluting sodium, which is its own cramp risk.

On sodium, the standard public health message is to eat less of it, and for someone eating a large processed diet that is sound advice. On a GLP-1, the context changes. When you have halved your food volume, you may have unintentionally dropped well below the amount your body needs to hold onto fluid. Unless your doctor has restricted sodium for blood pressure, heart failure or kidney disease, salting your food to taste is reasonable and often solves the problem on its own. Front-loading matters because by evening, fullness often makes a protein-forward dinner unrealistic.

Resistance training. Two to three sessions per week covering the major movement patterns is the most reliable protective factor for lean mass during weight loss. It does not have to be a gym. When are cramps a sign of something more serious?

The vast majority of cramps on a GLP-1 are benign and fixable. A small number are not, and it is worth knowing the difference so you neither panic nor dismiss something real.

Also worth flagging: severe or worsening cramps alongside ongoing vomiting or diarrhoea during dose escalation.
