Why does stroke risk increase after menopause?
Because several things that estrogen was quietly managing all start moving in the wrong direction at once.
A stroke happens when blood flow to part of the brain is interrupted. About 87% are ischemic — a clot blocks an artery. The rest are hemorrhagic — a vessel ruptures and bleeds. Most of what changes at menopause affects ischemic risk.
Estrogen acts directly on blood vessels. It promotes production of nitric oxide, which keeps arteries flexible and dilated. It has favorable effects on the vascular lining, or endothelium. When estrogen falls, arteries stiffen measurably — a change that can be detected on vascular testing years before any clinical event.
At the same time, three major risk factors worsen:
Blood pressure rises. SWAN and other cohorts have documented increases in both systolic blood pressure and arterial stiffness across the menopause transition. Hypertension is the single largest modifiable risk factor for stroke worldwide. Body composition changes. Visceral fat increases and insulin sensitivity declines, both of which raise vascular risk independent of body weight.

Does hormone therapy increase stroke risk?
It depends substantially on the route, the dose, and when you start — which is why the blanket answers you'll find online are usually wrong in one direction or the other.
What the WHI found. The Women's Health Initiative randomized more than 27,000 postmenopausal women to oral conjugated equine estrogen, with or without medroxyprogesterone acetate, or placebo. The estrogen-plus-progestin arm showed an increased risk of ischemic stroke, translating to roughly 8 additional strokes per 10,000 women per year. The estrogen-alone arm showed a similar direction of effect.
Those are real findings and they should not be waved away. But three caveats matter enormously. The average participant was 63 years old and more than a decade past menopause — not the typical woman starting hormone therapy for symptoms in her early fifties. The formulation was oral conjugated equine estrogen, not the transdermal estradiol most commonly prescribed today. And the absolute numbers are small: 8 per 10,000 per year is a real increase on a low baseline. Why route matters. Oral estrogen passes through the liver first, where it increases production of clotting factors. Transdermal estrogen — patch or gel — enters the bloodstream through the skin and bypasses that step. Large nested case-control analyses using UK primary care data have found that transdermal estrogen at standard doses is not associated with increased stroke or venous thromboembolism risk, while oral preparations are. Why timing matters. The timing hypothesis proposes that hormone therapy started near menopause, when arteries are still relatively healthy, has different vascular effects from therapy started a decade or more later on arteries that already have established plaque. What about migraine with aura?
This is the risk factor most often missed in midlife hormone conversations, and it genuinely changes the calculation.
Migraine with aura means migraine preceded or accompanied by neurological symptoms — most commonly visual disturbances like flickering lights, zigzag lines, or a blind spot, sometimes tingling or speech difficulty. Aura typically lasts five to sixty minutes before the headache begins.
Migraine with aura is associated with roughly a doubling of ischemic stroke risk in women, a finding replicated across multiple large cohorts including the Women's Health Study. The absolute risk in a healthy midlife woman remains low, but the relative increase is consistent and it compounds with other factors — combined hormonal contraception and smoking in particular.
This matters in perimenopause for a specific reason: migraine patterns often change during the transition, and some women develop aura for the first time in their forties. If that has happened to you, it should be part of your hormone therapy discussion, because guidance around estrogen differs between migraine with aura and migraine without it. Combined hormonal contraceptives are generally avoided in women with migraine with aura; menopausal hormone therapy at lower doses, especially transdermal, is treated differently — but the conversation should be explicit rather than assumed. One important distinction: migraine *without* aura does not carry the same stroke association. If you are unsure which you have, the presence of visual or neurological symptoms *before* the headache is the defining feature — and it is worth pinning down before a hormone decision rather than after.
Which risk factors can I actually change?
Most of them, and several have large effects.
Blood pressure is the highest-yield target. Hypertension is the leading modifiable contributor to stroke. Many women in midlife have never had it checked outside a rushed appointment, and blood pressure that rises quietly through the fifties is easy to miss. A home monitor costs little. If your readings sit consistently above 130/80, that's a conversation, not a curiosity.
Atrial fibrillation detection. AF is often intermittent and often silent, and it increases stroke risk substantially. Palpitations that come and go deserve investigation rather than reassurance — many consumer wearables now flag irregular rhythms, and a flagged reading is worth bringing to a clinician.
Smoking. The single largest voluntary risk multiplier, and its effect compounds with estrogen-containing medication.

Sleep apnea. Underdiagnosed in women, increases after menopause, and independently raises stroke and hypertension risk. If you snore, wake unrefreshed, or have morning headaches, ask about a sleep study.
Diet. Mediterranean-style eating has randomized trial evidence for cardiovascular event reduction, and stroke specifically was among the endpoints reduced in PREDIMED.
One group deserves particular attention: women who reached menopause before age 45, whether naturally or surgically. Early menopause is associated with higher cardiovascular and stroke risk later in life, and it is a reason for earlier and more thorough risk factor assessment — and often a reason to consider hormone therapy at least until the average age of natural menopause.
What are the stroke warning signs women miss?
The classic signs are essential to know — and women are more likely than men to present with symptoms that don't fit the classic picture, which contributes to delayed treatment.
The standard warning signs, using the BE-FAST framework:
Balance — sudden loss of balance or coordination. Eyes — sudden vision loss or double vision. Face — one-sided drooping. Arms — one-sided weakness or numbness. Speech — slurred or confused speech, or difficulty understanding others. Time — call emergency services immediately.
Symptoms more often reported by women include sudden severe headache with no known cause, sudden generalized weakness or fatigue, confusion or disorientation, nausea and vomiting, hiccups, chest pain, and shortness of breath. These are less specific, which is exactly the problem: they get attributed to a migraine, to anxiety, to perimenopause, or to being tired.
The rule that matters is suddenness. Perimenopausal symptoms build over months. Stroke symptoms arrive in seconds to minutes. Sudden onset of any neurological symptom — even one that resolves — is an emergency.
Do not wait to see if it passes. Clot-dissolving treatment for ischemic stroke has a narrow time window, and a mechanical clot retrieval window that is also limited. Every minute of delay costs brain tissue. Call emergency services rather than driving yourself or waiting for a partner to get home.
Transient symptoms still count. A transient ischemic attack — symptoms that resolve within minutes to hours — is a warning, not a false alarm. Stroke risk in the days immediately after a TIA is substantial, and urgent assessment meaningfully reduces it. "It went away" is a reason to go to hospital, not a reason to stay home.
