What actually helps physical discomfort during sex?

Local vaginal estrogen is the primary treatment for GSM. It comes as a cream, a tablet, a ring, or a suppository, and it works by restoring the vaginal lining, blood flow, and natural lubrication. Systemic absorption is minimal — blood estrogen levels stay in the postmenopausal range — which is why it is generally considered appropriate even for many women who cannot take systemic hormone therapy. Non-hormonal moisturizers and lubricants serve different purposes and both are worth having. A vaginal moisturizer is used regularly, two or three times a week, and rehydrates tissue over time. Both conversations are harder than they should be, and both go better with a small amount of preparation.
With a partner, the most useful move is to separate the physical from the emotional out loud. Unexplained withdrawal is almost always read as rejection or loss of attraction. *"My libido is low."* — likely to get a shrug and a comment about stress.
*"Sex has been painful for eight months, with burning at the entrance and some spotting afterward. I'm postmenopausal and on tirzepatide. Can we discuss local vaginal estrogen?"* — a clinical problem with a clear next step.
Name the symptom, name the duration, name what you've already tried, and name what you'd like to discuss. If your clinician dismisses it, ask for a referral to a menopause specialist. The Menopause Society maintains a directory of certified practitioners.
A few things worth mentioning at that visit: every medication you take, since SSRIs, some blood pressure drugs, and certain antihistamines all affect desire and lubrication; your current calorie intake, if you're on a GLP-1 and eating very little; and your sleep, honestly.
And give it time. Local estrogen typically needs six to twelve weeks for full effect. Pelvic floor therapy is a course, not a session. This is a fixable problem, but it is not a one-week fix.
