Why does perimenopause drain your iron?
Because the hormonal chaos of perimenopause frequently produces heavier, longer, and less predictable periods — and every period is an iron withdrawal.
In a regular cycle, estrogen builds the uterine lining and progesterone stabilizes it. In perimenopause, ovulation becomes irregular, which means progesterone production becomes irregular too. Estrogen can surge without the balancing effect of progesterone, and the lining grows thicker than usual before it sheds. The result is the pattern so many women in their forties describe: periods that arrive unpredictably, last longer, and are dramatically heavier than anything they experienced in their thirties.
Heavy menstrual bleeding is the single most common cause of iron deficiency in women of reproductive age, and perimenopause is when it peaks. Clinically, bleeding is considered heavy if you soak through a pad or tampon every hour or two, pass clots larger than a coin, need double protection, or bleed for more than seven days. Many women meet these criteria and never mention it, because they assume it is simply what perimenopause is.
The cruel timing is that iron requirements do not drop until menstruation actually stops. The recommended daily intake for menstruating women is 18 mg per day. After menopause it falls to 8 mg. Perimenopause is the window where you may be bleeding more than ever while still needing the full 18 mg — and it can last four to eight years.

How does a GLP-1 make low iron more likely?
By reducing how much food you eat, and by changing which foods you want to eat.
Total intake falls. GLP-1 medications work partly by slowing stomach emptying and increasing satiety, and the practical effect is that people eat substantially less. That is the point of the medication. But nutrients arrive in food, and when the volume of food drops by a quarter or a third, the volume of iron drops with it unless you deliberately restructure what you eat.
Red meat aversion is common. This is the specific problem. Many people on GLP-1s report that meat — particularly beef and other red meats — becomes unappealing, sits heavily, or triggers nausea. Red meat happens to be the richest source of heme iron, the form your body absorbs most efficiently. Heme iron from animal sources is absorbed at roughly 15 to 35%, while non-heme iron from plants, fortified grains, and supplements is absorbed at closer to 2 to 20%, depending heavily on what else is in the meal. Reflux medications compound it. Acid reflux is common on GLP-1s because of delayed gastric emptying, and many people end up on a proton pump inhibitor. Stomach acid is required to convert dietary non-heme iron into an absorbable form, so acid-suppressing medication meaningfully reduces iron absorption. If you are on both a GLP-1 and a PPI during perimenopause, you have three factors stacked.
An honest caveat: direct trial data on iron status specifically during GLP-1 therapy is still limited. That is not a reason to ignore it — it is a reason to measure rather than assume. What are the symptoms of low iron — and why do they get missed?
They get missed because every single one of them is also a symptom of perimenopause, a symptom of being on a GLP-1, or both.
The overlap is almost total:
- •Fatigue that sleep does not fix. Blamed on night sweats, blamed on eating less. Also the cardinal sign of iron deficiency.
- •Hair shedding. Blamed on estrogen decline, blamed on rapid weight loss. Low ferritin is an independent and well-documented cause of diffuse hair shedding in women.
- •Brain fog and poor concentration. Blamed on perimenopause. Iron is required for oxygen transport and for the synthesis of dopamine and other neurotransmitters.
- •Breathlessness climbing stairs. Blamed on deconditioning.
- •Cold hands and feet, pale inner eyelids, brittle or spooned nails. These get noticed less often but are more specific to iron.
- •Restless legs at night. Strongly associated with low ferritin, and frequently mistaken for menopausal insomnia.
- •Unusual cravings for ice or crunching on ice (called pagophagia). This one is oddly specific to iron deficiency and worth mentioning to your doctor if it applies to you.
This diagnostic overlap is precisely why iron gets overlooked. When a 47-year-old woman on a GLP-1 reports exhaustion and hair loss, there are two obvious explanations sitting right there, and a blood test never gets ordered.

Which blood test should you actually ask for?
Ferritin. Not just a complete blood count, and not just hemoglobin.
This distinction matters more than almost anything else in this article. Your body depletes iron in stages, and standard tests only catch the last one:
1. Stage one — storage depletion. Ferritin falls. Hemoglobin is completely normal. You may already feel exhausted and be shedding hair. 2. Stage two — reduced transport iron. Transferrin saturation falls. Hemoglobin still normal. 3. Stage three — anemia. Hemoglobin finally drops below the threshold, which for non-pregnant women is 12 g/dL.
A standard complete blood count only detects stage three. Someone can spend a year in stage one, feel awful, be told their bloodwork is fine, and have that be technically true.
The numbers to know:
- •Ferritin below 15 ng/mL — definite iron deficiency by any standard.
- •Ferritin below 30 ng/mL — widely accepted as indicating depleted iron stores in women, and the threshold most hematologists use.
- •Ferritin 30–50 ng/mL — a grey zone. Many clinicians treating hair loss or fatigue aim for above 50, though this is a clinical target rather than a diagnostic cutoff.
One important trap: ferritin is an acute-phase reactant, meaning it rises with inflammation, infection, and liver disease. So a ferritin of 60 in someone with significant inflammation may be masking real deficiency. If your ferritin looks borderline and your symptoms do not fit, ask for CRP alongside it, plus transferrin saturation.
What to ask for at your appointment, verbatim: "Can we check ferritin, a complete blood count, transferrin saturation, and CRP?" That set answers the question properly. How do you get enough iron when you can barely finish a meal?
By making the small amount of food you do eat work harder — and by paying attention to timing, because absorption is where most of the gains are.
Prioritize heme iron in small portions. You do not need a steak. Three ounces of beef, lamb, or dark-meat poultry delivers meaningful heme iron in a portion that fits a reduced appetite. Sardines, mussels, clams, and oysters are exceptionally iron-dense. Liver is the richest source of all, and even a small amount weekly counts. If meat is genuinely intolerable, that is common on a GLP-1 and worth planning around rather than forcing.
Pair non-heme iron with vitamin C. This is the highest-leverage habit in the whole article. Vitamin C can increase non-heme iron absorption several-fold. Lentils with a squeeze of lemon, fortified cereal with strawberries, spinach with bell peppers, beans with tomato and citrus. The vitamin C needs to be in the same meal, not later in the day.
Separate the blockers. Coffee, black tea, and green tea contain polyphenols that substantially inhibit iron absorption. So does calcium — which means your calcium supplement and your iron-rich meal should not share a table. Aim for at least one hour between tea or coffee and an iron-containing meal. On supplements. Do not start iron supplements without testing first. Excess iron is not harmless — it accumulates, and some people carry genetic variants (hemochromatosis) that make supplementation actively dangerous. If your ferritin comes back low, ask about alternate-day dosing: research suggests taking iron every other day can improve absorption compared with daily dosing, because a daily dose raises a hormone called hepcidin that blocks the next day's absorption. Alternate-day dosing also tends to be far gentler on the gut, which matters enormously when a GLP-1 is already slowing your digestion.
