What is inflammation, and why does it matter in midlife?

Inflammation is your immune system's repair response. Acute inflammation is useful and short-lived: you cut your finger, the area gets red and warm, and it heals. Chronic low-grade inflammation is different. It is a persistent, low-level activation of the same system with no injury to fix, and over years it damages blood vessel linings, interferes with insulin signaling, and accelerates bone loss.

Which inflammation test should you actually ask for?

The single most useful test is high-sensitivity C-reactive protein (hsCRP). It is cheap, widely available, and the version your doctor should order is specifically the high-sensitivity assay, not standard CRP. Standard CRP is designed to detect large inflammatory signals from infection. hsCRP detects the low-level range that predicts cardiovascular risk.

The generally used risk bands are:

  • Below 1 mg/L — lower cardiovascular risk
  • 1 to 3 mg/L — average risk
  • Above 3 mg/L — higher risk
  • Above 10 mg/L — likely an acute illness or injury, not baseline inflammation

That last point matters more than people realize. If you have a cold, a flaring joint, a recent dental infection, or a hard workout in the past 48 hours, your hsCRP will be elevated for reasons that have nothing to do with your long-term risk. A meaningful hsCRP is drawn when you are well, and ideally confirmed with a second reading two to four weeks later.

How much do GLP-1 medications lower inflammation?

Substantially, and the evidence comes from the largest trials in the field.

The SELECT trial (NEJM 2023) enrolled more than 17,000 adults with overweight or obesity and established cardiovascular disease, but without diabetes, and randomized them to semaglutide 2.4 mg or placebo. The headline result was a 20% reduction in major adverse cardiovascular events. Less publicized was the change in hsCRP: it fell by roughly 38% relative to placebo. That is a larger relative drop than most anti-inflammatory interventions produce.

STEP 1 (NEJM 2021), the pivotal weight-loss trial of semaglutide 2.4 mg, found a comparable reduction in hsCRP alongside average weight loss of 14.9% at 68 weeks. SURMOUNT-1 (NEJM 2022), which tested tirzepatide and produced 20.9% average weight loss at 72 weeks with the 15 mg dose, likewise reported improvements across inflammatory and metabolic markers.

The interesting question is mechanism. Some of the improvement is straightforward: fat tissue, especially visceral fat, is metabolically active and secretes IL-6 and other inflammatory signals. Lose the fat, lose the signal. But analyses of the cardiovascular benefit in SELECT suggested the risk reduction appeared earlier and was larger than weight loss alone would predict, which has led researchers to propose direct anti-inflammatory effects of GLP-1 receptor activation on blood vessel walls and immune cells.

Why does visceral fat matter more than the number on the scale?

Because visceral fat is the inflammatory tissue, and menopause specifically redistributes fat toward it.

Visceral adipose tissue is the fat packed around and between the abdominal organs, distinct from the subcutaneous fat you can pinch. It is densely populated with immune cells, and it drains directly into the portal vein that feeds the liver. That means the inflammatory signals it produces reach the liver at high concentration before diluting into general circulation.

SWAN data showed that visceral fat increases during the menopause transition even in women whose total body weight stays stable. The waistband changes before the scale does. This is why a woman can weigh what she weighed at 40 and still have a worse metabolic profile at 52.

Measure at the level of your navel, at the end of a normal exhale, standing relaxed. Does HRT raise or lower inflammation?

It depends almost entirely on the route of delivery, and this is one of the most clinically useful distinctions in menopause medicine.

What else moves inflammation while you are on a GLP-1?

Medication is one lever. These are the others, in rough order of how much evidence supports them.

Sleep. Short and fragmented sleep raises IL-6 and CRP measurably within days. Menopause makes this harder, between night sweats and the [insomnia that arrives with the transition](/blog/menopause-insomnia-why-you-cant-sleep-and-what-helps). Fixing sleep is not a soft recommendation here; it is one of the strongest non-drug levers available.

Resistance training. Muscle is anti-inflammatory tissue. It secretes myokines during contraction that oppose inflammatory signaling. Protein intake. Not directly anti-inflammatory, but adequate protein is what makes resistance training work. Most midlife women on a GLP-1 undershoot, often badly, because appetite is suppressed.

Oral health. Periodontal disease is a legitimate, measurable contributor to systemic hsCRP. Menopause increases gum inflammation risk, and dry mouth on a GLP-1 compounds it.

What does not reliably help: most anti-inflammatory supplements. Omega-3 has the best evidence of the group and it is modest. Turmeric and similar compounds have poor oral bioavailability and inconsistent trial results.

When should you retest, and what counts as a real improvement?

Retest hsCRP no sooner than three months after starting a GLP-1, and ideally at six months. Inflammation markers follow body composition change, and body composition change lags dose escalation.

A meaningful result is movement across a risk band — from above 3 mg/L down into the 1 to 3 range, or from that middle band down below 1. Small fluctuations within a band are noise. hsCRP has real biological variability week to week, which is why a single value is a weak data point and a trend across three readings is a strong one.

Draw the sample when you are well. No acute illness in the past two weeks, no dental work, no unusually hard training session in the prior 48 hours, and ideally not during a flare of any chronic inflammatory condition.

If your hsCRP stays above 3 mg/L despite meaningful weight and waist reduction, that is worth investigating rather than ignoring. And a caution about interpretation: hsCRP is a risk marker, not a diagnosis. Lowering it is associated with better outcomes, but nobody should treat the number in isolation.
