Heart Palpitations in Menopause: Causes, Red Flags and Tests

Up to 40% of midlife women get palpitations. Here's what causes them, the red flags that need urgent care, and the tests worth asking for.

How common are palpitations in menopause?

Far more common than most women are told, and far less discussed than hot flashes.

Analyses from the Study of Women's Health Across the Nation (SWAN), the large longitudinal cohort that has followed midlife women since 1996, have found that roughly 20 to 40 percent of midlife women report palpitations, with prevalence varying by how the question is asked and over what timeframe. Carpenter and colleagues, working with SWAN data, have documented that palpitations are consistently reported alongside vasomotor symptoms rather than in isolation.

What women describe varies:

  • A single hard thump, often at rest or lying down
  • A fluttering or "flip-flop" feeling in the chest or throat
  • A brief run of fast beats
  • A sensation of the heart skipping, then a compensatory strong beat
  • An awareness of the heartbeat that simply was not there before

Most of these correspond to ectopic beats, meaning premature atrial or ventricular contractions. These are extra beats that arise outside the normal pacemaker rhythm. Almost everyone has them. What changes in midlife is that women start *noticing* them, and that they become more frequent.

The timing is telling. Palpitations cluster in perimenopause rather than late postmenopause, which is the same pattern as hot flashes, migraines and mood symptoms. It is the volatility of hormone levels, not the eventual low level, that seems to drive the symptom.

The part that deserves naming: palpitations are one of the symptoms most likely to be dismissed. Women describing them are more often told it is anxiety, and more often told it without a workup. Anxiety is a genuine and common contributor, but it is a diagnosis of exclusion, not a first assumption.

Why does menopause cause palpitations?

The leading explanation is a shift in autonomic nervous system balance driven by falling and fluctuating estrogen.

Your heart rate is governed by the balance between the sympathetic branch (fight or flight, which speeds things up) and the parasympathetic branch (rest and digest, which slows things down). Estrogen influences both. It modulates vascular tone, affects the sensitivity of adrenergic receptors, and appears to support parasympathetic activity. As estrogen falls and swings unpredictably, the balance tips toward sympathetic dominance. The measurable result in research settings is reduced heart rate variability, a marker of autonomic flexibility that declines across the menopause transition.

Three secondary mechanisms compound it:

Vasomotor coupling. A hot flash is itself an autonomic event, involving a surge in skin blood flow and often a rise in heart rate of 7 to 15 beats per minute. Many women who describe palpitations are describing the cardiac component of a flush they did not otherwise register, particularly at night.

Sleep fragmentation. Poor sleep raises circulating catecholamines and lowers the threshold at which you notice an extra beat. Night sweats and palpitations frequently arrive together for exactly this reason. The anxiety loop. Noticing your heartbeat causes alarm. Alarm raises adrenaline. Adrenaline produces more ectopic beats and more awareness of them. This is a real physiological loop, not a dismissal, and breaking it is a legitimate treatment target. When should you worry about palpitations?

Most midlife palpitations are benign. Some are not, and the distinguishing features are specific enough to be worth memorizing.

Seek urgent medical care if palpitations come with:

  • Fainting or near-fainting. Losing consciousness with palpitations is the single most concerning combination and needs same-day assessment.
  • Chest pain or pressure, particularly if it radiates to the jaw, neck, back or arm.
  • Severe breathlessness out of proportion to activity.
  • Palpitations that start during exertion rather than at rest. Benign ectopics typically appear at rest and often disappear when you get moving. The reverse pattern is a warning sign.

Book a non-urgent appointment if you have:

  • Episodes lasting more than a few minutes, or a sustained rapid rhythm you can count as clearly abnormal
  • Palpitations that are irregular and disorganized rather than the isolated-thump pattern
  • A family history of sudden cardiac death, inherited arrhythmia or cardiomyopathy
  • Known structural heart disease or a prior cardiac event
  • Palpitations that are new, frequent, and interfering with your life

There is one arrhythmia that deserves specific mention. Atrial fibrillation becomes progressively more common with age, and women's symptoms often differ from the textbook presentation. Instead of a dramatic racing heart, women more often report fatigue, breathlessness on exertion and a vague unwellness, with the irregular rhythm only noticed incidentally. A practical tool: most modern smartwatches can record a single-lead ECG. If your episodes are intermittent and never happen in the clinic, capturing one during a symptom is genuinely useful data to bring in. Do not use it to self-diagnose, but do use it to document.

What tests should you ask for?

A first episode of new palpitations in midlife deserves a basic workup, and it is inexpensive. Four tests cover most of the ground.

12-lead ECG. Ten minutes, widely available. It captures your rhythm at that moment and, importantly, screens for conduction abnormalities and prior silent damage even when you are not symptomatic.

Thyroid function (TSH, and free T4 if TSH is abnormal). Both an overactive and an underactive thyroid cause palpitations, and thyroid disease is substantially more common in midlife women. It also mimics almost every other perimenopausal symptom, which is why it is missed so often. Full blood count and ferritin. This one is frequently skipped and frequently the answer. Heavy, unpredictable perimenopausal bleeding is a leading cause of iron deficiency in midlife women, and anemia causes palpitations, breathlessness and fatigue. Ask for ferritin specifically, not just hemoglobin, because iron stores can be depleted long before hemoglobin drops.

Basic metabolic panel including electrolytes. Low potassium and low magnesium both increase ectopic beats and are easily corrected.

If episodes are frequent or the basic workup is unrevealing, the next step is ambulatory monitoring: a 24 to 48 hour Holter monitor, or a patch or event monitor worn for one to two weeks if episodes are less frequent. The longer monitors are far more likely to catch an intermittent arrhythmia. An echocardiogram is added if there is any suspicion of structural heart disease, a murmur, or an abnormal ECG.

One advocacy note worth stating directly. Research has repeatedly documented that women presenting with cardiac symptoms are less likely to be referred for further testing and more likely to have symptoms attributed to anxiety. If you have been told it is anxiety without an ECG and blood tests, it is entirely reasonable to ask, in writing, for those to be done and for the result to be recorded.

What actually helps benign menopausal palpitations?

Once serious causes have been excluded, the useful interventions fall into three tiers.

Tier 1: remove the amplifiers. These are boring and they work.

  • Caffeine. Individual sensitivity varies enormously, but caffeine reliably increases ectopic beats in susceptible people. A two-week trial of cutting back tells you more than any general advice.   • Dehydration. Low circulating volume raises heart rate and makes ectopics more noticeable.
  • Poor sleep. The highest-yield intervention for most women, and the hardest.
  • Stimulant medications and decongestants. Pseudoephedrine in cold remedies is a common unrecognized trigger.

Tier 2: address the autonomic pattern. Slow diaphragmatic breathing, particularly extended-exhale patterns, measurably raises parasympathetic tone. Regular aerobic exercise improves heart rate variability over weeks. Cognitive behavioural approaches are effective for the noticing-alarm loop specifically, and CBT has a strong enough evidence base in menopause that it is recommended in UK NICE guidance for several symptom domains.

Tier 3: medical options.

Hormone therapy is not indicated for palpitations on their own. However, where palpitations are clearly tied to vasomotor symptoms, women who start HRT for hot flashes frequently report the palpitations settling too. The evidence here is observational and secondary rather than from trials designed to test it, so treat it as a plausible bonus rather than a reason to start.

Beta blockers at low dose are sometimes prescribed for symptomatic benign ectopics. They blunt the adrenergic response and reduce awareness of the beats. They do not fix the underlying cause and have their own trade-offs, including fatigue, so they are a symptom-management tool rather than a solution.

Magnesium correction is worthwhile if you are deficient. Supplementing beyond correction has weak evidence for palpitations specifically.

One thing that is genuinely reassuring and worth internalizing: benign ectopic beats in a structurally normal heart do not damage it. The sensation is unpleasant and frightening. It is not doing harm. For many women, a clear ECG plus that piece of information reduces the frequency of episodes by itself, because it takes the alarm out of the loop.
