Why are so many people microdosing in 2026?
People microdose for four practical reasons, and none of them is that a study told them to. Understanding the actual motivations matters, because for two of the four there is a better-evidenced alternative.
Reason one: side effects. Nausea, reflux, constipation and fatigue cluster during titration and dose escalation. Rather than push through, some people stop climbing and stay low. What does the research actually say about low-dose GLP-1s?
The honest answer is that no randomized controlled trial has tested microdoses for weight loss, inflammation, longevity or metabolic health. What we have instead is the shape of the dose-response curve from the approved trials, and it points in an unhelpful direction for microdosing.
STEP 1 (NEJM 2021) tested semaglutide 2.4 mg against placebo in 1,961 adults and found 14.9% average weight loss at 68 weeks versus 2.4% on placebo. SURMOUNT-1 (NEJM 2022) tested tirzepatide and found 20.9% at 15 mg at 72 weeks, with the 5 mg arm coming in lower at around 15%. Across the GLP-1 programme, the pattern repeats: more drug, more effect. There is no plateau in the low range that would let a microdose deliver most of the benefit.

Is microdosing a GLP-1 safe?
The safety concern with microdosing is usually not the dose. It is the supply chain.
A smaller amount of semaglutide is, pharmacologically, less drug — so the classic dose-related side effects (nausea, vomiting, diarrhoea, gallbladder issues) are generally less likely, not more. That is the genuine upside people are chasing.
The risk sits elsewhere. Because approved pens come in fixed dose increments, almost all microdosing runs through compounded products or through people drawing custom amounts out of a vial themselves. That introduces three specific problems:
Potency uncertainty. Compounded preparations are not reviewed by the FDA for potency, purity, or sterility the way approved products are. If you cannot verify concentration, "0.1 mg" is an intention rather than a measurement. Measurement error. Drawing a fraction of a unit from a vial with an insulin syringe leaves very little margin. A small volume error that would be trivial at 2.4 mg is proportionally enormous at 0.1 mg — and the error can run either direction.
Salt forms and unapproved variants. During the shortage, some compounded products used semaglutide sodium or semaglutide acetate — salt forms the FDA has explicitly warned are not the approved active ingredient and have not been shown to be safe or effective.
There is also an opportunity cost that rarely gets named. Someone who spends a year on a microdose, loses 3% of their body weight, and concludes "GLP-1s don't work for me" may have simply never taken a dose that was ever expected to work. That is a meaningful clinical loss, not just a financial one.
What questions should you ask before microdosing?
If you are considering a microdose, five questions will tell you more about whether it is a reasonable plan than any amount of reading about mechanisms.
1. What specifically am I trying to achieve, and how will I know if it worked? This sounds obvious and it is the question most often skipped. "Feel better" and "optimise my metabolism" are not measurable. "Lose 8% of my body weight," "bring my HbA1c under 5.7," or "maintain the 40 pounds I already lost" are. Set the target and the review date before you start, because a vague goal on an unproven dose can absorb a year without ever producing a decision.
2. Where is the product coming from, and can the pharmacy verify potency? Ask for a certificate of analysis. A compounding pharmacy that cannot or will not provide one is telling you something. Also ask specifically whether the product is semaglutide base or a salt form — the FDA has warned that semaglutide sodium and semaglutide acetate are not the approved active ingredient.
3. Who is supervising this? A prescriber who sees your labs, knows your history, and can escalate or stop is a materially different situation from a telehealth form that ships a vial. Microdosing is not inherently unsafe, but unsupervised self-titration from an unverified vial is a different proposition entirely.
4. Have I actually tried the evidence-based path? If the reason for microdosing is side effects, have you tried holding at each titration step for six or eight weeks instead of four? If it is cost, have you priced the branded cash-pay programmes? A lot of microdosing is solving a problem that has a better-tested solution.
5. What is my exit criterion? Decide now what result at three or six months would make you stop, escalate, or continue. Writing it down in advance is the difference between an experiment and a habit.
When does a lower dose genuinely make sense?
There are situations where staying at a lower dose is a sound clinical decision — they are just narrower than the microdosing conversation implies, and they mostly involve a doctor rather than a marketing page.
You have reached your goal and you are maintaining. This is the strongest case. Some people hold their weight on a dose below the maximum, and the maintenance question is a legitimate one to work through with a prescriber. It is also a very different scenario from starting low: you have already had the full-dose effect, and you are testing how little you need to keep it. You get real benefit at a lower dose and genuinely cannot tolerate more. Some people respond strongly to 0.5 or 1 mg. If you are losing weight and your metabolic markers are improving, there is no prize for escalating further. "Lowest effective dose" is standard medicine — but note the word *effective*. You should be able to point to a result.
You are tapering deliberately. Coming off a GLP-1 by stepping down, under supervision, is a reasonable strategy for managing appetite rebound.
Where it does not make sense: starting at a microdose hoping to reach a weight-loss goal, using microdoses for longevity or inflammation in someone who is not overweight, or self-titrating from a compounded vial without any clinical oversight.
