What is a coronary calcium scan?
It is a short, non-contrast CT scan of the heart that measures calcified plaque in the coronary arteries.
Here is why calcium is a useful thing to measure. Atherosclerosis — the buildup of plaque in artery walls — begins as soft, lipid-rich deposits. Over years, some of that plaque calcifies. Calcium is dense and shows up clearly on CT, so quantifying it gives a direct measure of how much atherosclerosis you have actually accumulated, rather than an estimate of how much you are likely to have based on your risk factors.
The output is an Agatston score, a number derived from the area and density of calcified deposits. It ranges from 0 upward, with no fixed ceiling.
The practical experience is undramatic. You lie on a scanner bed for about 10 minutes, with ECG leads on your chest so the machine can time images between heartbeats. There is no contrast dye, no IV, no fasting, and no recovery period. Radiation is around 1 millisievert, roughly comparable to a mammogram and a fraction of a standard chest CT.
The critical limitation to understand upfront: CAC measures calcified plaque only. Soft, non-calcified plaque — which is younger and in some ways more prone to rupture — does not appear. This is why a score of zero is reassuring about your near-term risk but is not a guarantee of clean arteries, and it is a particularly important caveat for women under 50.
Why does this matter more for women in menopause?
Because the standard tools for estimating women's cardiovascular risk are weakest exactly when the risk starts rising.
Cardiovascular disease is the leading cause of death in women, and risk climbs sharply after menopause. The SWAN study, following a large multi-ethnic cohort through the transition since 1996, found that a cluster of risk markers — LDL, apolipoprotein B, arterial stiffness, and visceral fat — accelerate around the final menstrual period specifically, not gradually with age. The transition itself is an inflection point.
Standard risk calculators handle this poorly. The commonly used equations were derived and validated largely in cohorts where men drove most events, and they weight age heavily. A 52-year-old woman almost always lands in a low or borderline category on calculated 10-year risk, even with a concerning profile, simply because she is a woman in her early fifties.
They also omit risk factors specific to women, which have solid evidence behind them:

If any of those apply to you, a calculator output of "low risk" is less trustworthy than it looks. A CAC scan bypasses the estimate entirely and measures what is actually there. What do the scores actually mean?
The score bands are well established and map onto fairly clear management implications.
CAC = 0. No detectable calcified plaque. This is the most actionable result, because it indicates very low risk of a cardiac event over the following five to ten years. It is sometimes called the "power of zero" and is often used to justify deferring or declining statin therapy in someone at borderline or intermediate calculated risk. The caveat stands: zero does not exclude soft plaque, and it carries less weight under age 50.
CAC 1–99. Mild plaque. Generally places you above average for your age and supports starting preventive treatment, particularly if other risk factors are present.
CAC 100–399. Moderate plaque. In most guidelines this alone is sufficient to recommend statin therapy regardless of what the calculator said.
CAC 400+. Extensive plaque. High risk, and usually prompts aggressive lipid management and a broader cardiac assessment.
One further refinement matters for women: the percentile for your age and sex. A score of 50 means something very different in a 45-year-old woman than in a 70-year-old man. A 45-year-old woman with any calcium at all is often above the 90th percentile for her demographic, which is a stronger signal than the raw number suggests. Ask for the percentile, not just the Agatston score.
What a CAC scan does not tell you: whether you have a blockage causing symptoms. It is a risk-stratification tool for people without symptoms. Who should get one, and who should not bother?
The scan is most valuable when the result could plausibly change what you do next. That means intermediate risk.
Strong candidates:
- •Women aged roughly 45 to 70 with a calculated 10-year risk in the intermediate range
- •Women genuinely undecided about starting a statin and wanting more information
- •Women with a family history of premature coronary disease — a first-degree male relative before 55, or a female relative before 65
- •Women with a female-specific risk factor the calculator ignores, such as preeclampsia or early menopause
- •Women with [rising LDL after 45](/blog/menopause-and-cholesterol-why-ldl-rises-after-45) who want to understand whether it has translated into plaque
Less useful:
- •Very low risk with no risk factors and a clean family history — the result will almost certainly be zero and change nothing
- •Already high risk, or established cardiovascular disease — you should already be on treatment, and the scan will not alter that
- •Already on a statin — calcium can paradoxically increase on statin therapy as soft plaque stabilizes and calcifies, which makes serial scores hard to interpret
- •Symptomatic chest pain — you need a different test, promptly
- •Under 40 — calcium has usually not had time to develop, so a zero is minimally informative
There is also a question of what you will do with the answer. The scan is most valuable for people who would genuinely act on a high score and would genuinely feel reassured by a zero. If a high score would only generate anxiety without changing behaviour, and a zero would not change your prevention plan either, the value is limited. That is a reasonable thing to think about honestly before booking.
What does it cost, and is it covered?
In the United States, a CAC scan typically costs $100 to $400 out of pocket. Many hospital systems and imaging centers offer it as a flat-rate self-pay service precisely because insurance coverage is inconsistent, and self-pay is frequently cheaper than going through insurance.
Coverage varies considerably. Some plans cover it for intermediate-risk patients with a physician order; many do not cover it as a screening test. Texas and a handful of other states have passed legislation mandating coverage in specific circumstances. It is worth calling your plan and asking specifically about CPT code 75571.

A note on direct-to-consumer scanning: several companies market CAC scans without a physician order. The scan itself is identical, but the value of the result depends heavily on interpretation in the context of your full risk picture. Also worth knowing: the scan images the chest, so incidental findings in the lungs or elsewhere occur in a small percentage of scans. Most are benign, but they can lead to follow-up imaging. That is a real, if modest, downside of any CT-based screening.
What should you do with the result?
The scan is a starting point for a conversation, not a verdict.
If your score is zero, you have strong evidence of low near-term risk. This may support deferring a statin, with a repeat scan in five years if risk factors persist. It does not mean you can ignore blood pressure, lipids, or lifestyle — it means you have time. Under 50, treat a zero as encouraging rather than definitive, since soft plaque does not show.
If your score is elevated, the main action is lipid-lowering therapy, usually a statin, plus attention to blood pressure. The reason to act is straightforward: plaque that exists is plaque that can progress. This is treatable, and treating it early is the entire point of having looked.

Glucose and insulin resistance. Visceral fat.
